Disclaimer

ASSUMPTION OF RISK: I have read and understood the above warning concerning COVID-19. I hereby choose to accept the risk of contracting COVID-19 for myself and/or my child(ren) in order to utilize Life Long Tennis Academy services. These services are of such value to me and/or my child(ren) that I accept the risk of being exposed to, contracting, and/or spreading COVID-19 in order to utilize Life Long Tennis Academy  serves in person. 

WAIVER OF LAWSUIT/LIABILITY: I hereby forever release and waive my right to bring suit against Life Long Tennis Academy  and its agents and employees, volunteers, board members and other representatives in connection with exposure, infection and/or spread of COVID-19 related to utilizing Life Long Tennis Academy  services and premises. I understand that this waiver means I give up my right to bring any claims including for personal injuries, death, disease or property losses, or any other loss, including but not limited to claims of negligence and give up any claim I may have to seek damages whether known or unknown, foreseen or unforeseen.

I hereby give my permission for my child to participate in Life Long Tennis Academy  educational activities. I understand that sports activities could include play and outdoor activities around and near the sports grounds or natural environments where it can be slippery and rough surfaces among other dangers and risks.

I understand that Life Long Tennis Academy  may, in his sole discretion, dismiss any lesson participant for inappropriate, disrespectful, or dangerous behavior at any time. In this event, I understand that I will not receive a refund of lesson fees for unattended days.

If my child breaks or damages any property as a result of their direct or indirect behavior, I hereby agree to pay for its repair or replacement. I understand that the risks associated with lesson activities could result in injury and/or death to my child.

I hereby assume these risks and, knowing them, hereby give my child permission to participate.

Because physical exercise can be strenuous and subject to risk of serious injury, we urge you to obtain a physical examination from a doctor before using any exercise equipment or participating in any exercise activity.

I understand that I am voluntarily participating in these activities and use of these facilities and premises and assume all risks of injury, illness, or death.

Life Long Tennis Academey  is also not responsible for any loss of my personal property. I acknowledge that you have carefully read this “exercise disclaimer” and fully understand that it is a release of liability.

I expressly agree to release and discharge Life Long Tennis Academy  from any and all claims or causes of action and agree to voluntarily give up or waive any right that I may otherwise have to bring a legal action against Life Long Tennis Academy  for personal injury or property damage.

Photo Waiver Release Form

I hereby grant the releasee permission to use photographs in any of the following:

I hereby affirm that such release to the releasee does not constitute any form of compensation, including royalties arising from the photographs, to my benefit.

I understand and agree that photographs in the possession of the releasee shall become the property of the releasee. The use and publication of the photographs however, shall conform to my rights as a subject of said photographs.

I hereby waive my right to inspect of approve the photographs by which my likeness appears.

I hereby hold harmless, release, and forever discharge the  from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization. I declare true my age of consent, and in case I am under the age of such consenting age, I have obtained the required consent from my parents/guardians as evidenced by the signatures below:

I HAVE CAREFULLY READ AND FULLY UNDERSTAND ALL PROVISIONS OF THIS RELEASE, AND FREELY AND KNOWINGLY ASSUME THE RISK AND WAIVE MY RIGHTS CONCERNING LIABILITY AS DESCRIBED ABOVE.
